Individualized Home Supports with Training

Individualized home support is provided to patients who live independently but require some assistance to do so. Generally, a patient receiving this program get training, assistance, and supervision in these areas:

  • Community participation
  • Health, safety, and wellness
  • Household management
  • Adaptive skills

Individualized Home Supports with Family Training

When a patient is living with his or her family, individualized home support is designed to train the family members to increase their capabilities in maintaining the support the patient needs to live at home. The family will be given training in at least one of the community living services categories.
